Description You failed to give effect to arrangements for the effective planning, organisation, control, monitoring and review of the preventive and protective measure for the decommissioning activities of the Little Dotty Well 48/30-09 from the Valaris 72 jack-up drilling rig, resulting in failure of the landing string crossover, a release of approximately 480kg of gas, and open communication between the reservoir and the environment for approximately 18 days, in that you did not: • Eliminate risks through the design of the crossover, in that you attempted to reverse engineer a crossover piece between the landing string and wellhead instead of using an Original Equipment Manufacturer’s supplied crossover. The thread run on the reverse engineered crossover was a standard 1” stub ACME opposed to the Cameron Ironworks proprietary thread specified in the Rig Book for the Little Dotty Xmas Tree, which contributed to the landing string stalling during installation without being fully engaged • Provide instructions that sufficiently highlighted critical tasks based on available relevant information included sufficient verification checks that those critical tasks had been successfully completed, or gave sufficient direction for the installation of the 9-5/8” landing string if critical task steps were not successfully completed. • Conduct an investigation that sufficiently considered all the causes of the incident, including both technical factors related of the design of the reverse engineered crossover piece and non-technical factors such as decision making of those involved, the quality and adequacy of written instructions and the quality of record keeping related to the work activity being undertaken.
