| Description | On the 5th August 2025, a worker was manually lowering a scaffold bag containing chain blocks with a mass of approximately 25 kgs to the floor of the pump room using a length of hemp rope that had been formed by tying two shorter pieces together. The rope parted and the scaffold bag and contents fell to the Pump room floor, a distance of approximately 25 metres to the pump room floor below, narrowly missing the mechanical technician working in the pump room. This length of heaving rope was not certified and patently not fit for the purpose that it was being used. The Mechanical technician in the pump room where the scaffold bag landed was exposed to the risk of being struck by this falling object and hence at risk of serious personal injury. He was unaware that an object load of such weight was to be lowered to the pump room floor. Your Control of Work process had not been effectively implemented in planning and carrying out this work, there was no reference to the task in a permit to work (PTW), nor any risk assessment nor a specific procedure that addressed carrying out of this work safely. The installation supervision were not aware that this task was to be undertaken at this time. Furthermore your measures in place to ensure that work was adequately planned, organised, carried out, monitored and reviewed have failed to ensure this task was carried out safely. | ||
