Description There is reason to suspect that the assessment undertaken under Regulation 3(1) of the Management of Health and Safety at Work Regulations 1999 is no longer valid because there was a liquid nitrogen release on 12 July 2025 that caused a failure of a section of deck which fell onto a walkway below and had fatal injury potential. Our investigation has identified underlying causes relating to the adequacy of your existing arrangements and/or adherence to those arrangements such as loss of containment events, suitability of weep and seep bunding, plant integrity checks, response to leaks, contractor management, control of work, monitoring, audit and review.
