| Description | My investigation regarding the unintentional release of hydrocarbon gas that occurred on the Lancelot 48/17A normally unattended installation (NUI) on 5th June 2025 identified that you have failed to make and give effect to appropriate arrangements for the effective planning and organisation of the permit-to-work system (described by PUK-SMS-OWC-029 Rev 19) in relation to operations on NUIs conducted using walk to work (W2W) vessels. Effective communication and control arrangements (such as the daily planning and permit meeting specified in PUK-SMS-OWC-029) were not established on the W2W vessel therefore omissions during work planning (such as failing to sufficiently identify hazards) were not addressed prior to execution. For NUIs where there is no associated manned installation the organisational roles and responsibilities are unclear and inconsistent with those specified in PUK-SMS-OWC-029. Consequently, workload and conflicting priorities are impacting upon the ability of responsible persons to properly carry out their duties within the system. | ||
