| Description | You have failed to take appropriate measures with a view to preventing fire and explosion on the Culzean installation in that you have not implemented your system of planned preventative maintenance for critical check valves which are installed and provide a safety critical function to restrict the maximum potential reverse flow as a basis for pressure relief device sizing, and you have not assessed and mitigated the risks by not having done so. Culzean started operations in 2019. Your Management of Critical Check Valves document, L2-OPS-14-021, dated 10 November 2021 at Revision 1 provides instruction on the management of such valves and the in-situ testing requirements, and where in-situ testing cannot be carried out a programme of replacement or internal inspection and reinstatement should be in place. The only maintenance record found was for critical check valve 0420-V0069 which was replaced in August 2022 because it had siezed. You have identified that critical check valves are Safety and Environmental Critical Elements (SECEs) and included in your P10 (Pressure Relief) Performance Standard. | ||
