Description 1st IN issued because you have failed to ensure that measures are taken which are effective to prevent access to the dangerous parts of the single-end tenoning machine, in that it is possible to access the top and bottom tenon cutting head and the scribing cutting heads beyond that what is required for the machinery to perform its cutting function, where access to these cutters can lead to lacerations and potentially amputation. 2nd IN issue because you have failed to ensure that the exposure of your employees to wood dust, a substance hazardous to health, is either prevented or where this is not reasonably practicable adequately controlled, as appropriate respiratory protective equipment has not been provided and inappropriate cleaning processes create secondary exposures to wood dust e.g., dry sweeping with brooms and using compressed air to remove settled wood dust. 3rd IN issued because you have failed to ensure that employees who are or are liable to be exposed via inhalation and skin contact to wood dust, a substance hazardous to health, are placed under suitable health surveillance where it is appropriate for the protection of their health e.g., when exposure can lead to occupational asthma and occupational dermatitis. 4th IN issued because you have failed to ensure that the risk from the exposure of your employees to noise is either eliminated at source or, where this is not reasonably practicable, reduced to as low a level as is reasonably practicable, as low noise cutters have not been installed on the business’s planer. 5th IN issued because you have failed to ensure that where there is a risk to the health of your employees exposed to noise e.g., employees exposed to daily noise at or exceeding 85dB(A), that they are placed under suitable health surveillance, which includes the testing of their hearing. 6th IN issued because as the user of an installed pressure system operating at 250 bar litres of pressure, consisting of a Gtec air compressor and welded pressure vessel, that you have allowed it to be operated before a written scheme for its periodic examination has been drawn up by a competent person.
