Description 1st IN issued because you have failed to ensure that measures are taken that are effective to prevent access to the dangerous part of the business’s table saw on the ground floor. It is possible to access the circular saw blade from the top of the blade as a crown guard has not been installed and this could lead to serious lacerations and potentially amputation. 2nd IN issued because you have failed to ensure that measures are taken which are effective to prevent access to the dangerous part of the business’ table saw in the basement, in that it is possible to access the circular saw blade from the top of the blade as a crown guard has not been installed and this can lead to serious lacerations and potentially amputation. 3rd IN issued because you have failed to provide a suitable and sufficient washing facility in the immediate vicinity of the business’ sanitary convenience, as this washing facility does not include a supply of clean hot and cold, or warm, running water (which shall be running water so far as practicable) where this is required for the facility to be considered suitable. 4th IN issued because you have failed to ensure that respiratory exposure to wood dust, a substance hazardous to health, is either prevented or where this is not reasonably practicable adequately controlled. Local exhaust ventilation (LEV) has not been provided for all woodworking equipment, large amounts of wood dust have been allowed to accumulate, and suitable respiratory protective equipment (RPE) has not been provided. 5th IN issued because you have failed to ensure that employees who are or are liable to be exposed to substances hazardous to health are placed under suitable health surveillance where it is appropriate for the protection of their health, as you workers are exposed to wood dust via inhalation and skin contact, where exposure to these substances can lead to occupational asthma and occupational dermatitis. 6th IN issued because you have failed to ensure that local exhaust ventilation systems attached to wood working machinery (the edge bander and table saw) that represent engineering controls for exposure to wood dust (a substance hazardous to health) have undergone a thorough examination and test within the last 14 months.
