Description | My inspection of Triton between 4th – 7th March 2024 identified that the alarm system performance remains significantly above the criteria stated by industry recognised guidance and your own alarm management standard in that there is a high number of standing alarms (835 vs target of <10) which has remained consistently high for at least 6 years, average or peak alarm rate per 10 minute period target has not been met for the past 6 months and the top 10 ‘bad actor’ alarm count per month consistently exceeds 100,000 with a high proportion of these alarms being designated as urgent or high priority. Alarms on your system are identified as an independent protection layer which were not highlighted to your workforce, and you did not provide a defined alarm response. You also have unresolved discrepancies between your DCS and Honeywell Dynamo Manager systems providing inconsistent real time data on alarm system performance. You received previous enforcement action in this area in 2018 and 2022 and as such have failed to make and give effect to such arrangements as are appropriate, having regard to the nature of your activities and the size of your undertaking, for the effective planning, organisation, control, monitoring and review of the preventative and protective measures. | ||
