| Description | Following the dropped riser package incident on 1st February 2024, as part of ongoing enquiries, a visit was made to the Ocean GreatWhite on 12th February 2024. During this visit I identified the controlling procedure for the assembly of a riser package was SSP 138-SUB-364 Rev. 7 Dated 04-Dec-2023 - Deployment of Tension Ring & Hands Free Goose Neck. On inspection of this document I found that the procedure contained no specific roles and responsibilites of persons within the company who were to perform the various actions required for the assembly of the riser package. Also the procedure did not contain any verification or assurance checks to confirm that the requirements of the procedure had been complied with, including such critical operations as locking the tension ring to the outer barrel of the slip joint. In addition there were no records maintained to demonstrate that the work had been carried out to the requirements of the procedure. Therefore you have failed to provide comprehensible instructions to ensure that the riser package is assembled correctly, the necessary levels of verification and assurance checks have been identified, records to be maintained and those involved have clearly stated roles and responsibilities. | ||
