| Description | You have failed to ensure, so far as is reasonably practicable, the safety of your employees and persons not in your employment during lifting operations with your offshore main cranes (model S72), in that two separate events have occurred where the auxiliary wire rope has been displaced from your boom tip sheave i.e., “running off the sheave” during operation. Firstly on the Valaris 121 Starboard S7220 crane on 25 Jan 2024 and on the Ensco 120 Port Forward S7220 Crane on 24 Feb 2024. In both cases this led to the deterioration of the said wire ropes, increasing the risk of a dropped object. The exact causation is unknown at this time and given the circumstances of having a similar failure and dropped object in 2020 on the same model of crane, there is an increased risk of a dropped object. | ||
