| Description | I carried out an investigation of a lifting incident that occurred on 18 December 2023. My investigation revealed that you failed to have effective arrangements in place for ensuring that lifting operations are appropriately supervised and carried out in a safe manner, in that 1) You failed to manage the barriered lifting area during the lifting of tubulars from the pipe deck to the drill floor as there were other personnel working within the area unknown to the lifting crew; 2) the banksman was working at height without a valid permit; 3) The split shift lifting crew did not have a suitable and sufficient pre-shift meeting or tool box talk; 4) radio communication between the banksman and the crane operator were not suitable and sufficient to have prevented the activation of the gripper whilst he was in an unsafe area, and upon its activation he was injured. | ||
