Description PN served - use of the knockout shakeout machine prohibited as Respirable Crystalline Silica (RCS) is a known substance hazardous to health which presents a risk of silicosis, chronic obstructive pulmonary disease and lung cancer. IN1/ You have failed to appoint one or more competent persons to assist you in undertaking the measures you need to take to comply with the requirements and prohibitions imposed upon you by or under the relevant statutory provisions, in that you do not have a person(s) with sufficient resource, skills, training and knowledge to manage the risks on your site. IN2/ You have failed to either eliminate or reduce, so far as is reasonably practicable, the risk of fire and explosion from dangerous substances, namely aluminium dust. Aluminium dust is combustible and an explosion hazard and there is evidence of dust build up in production areas within the foundry, such as fettling and finishing. IN3/ You have failed to ensure that the exposure of your employees to substances hazardous to health, namely molten metal fume is either prevented or, where this is not reasonably practicable, adequately controlled during melting, pouring and casting activities of molten metal. Health risks from fume include metal-fume fever and irritation of the eyes, nose and throat. Longer term exposure has been associated with serious lung diseases (e.g. aluminosis). IN4/ You have failed to ensure that the exposure of employees to a substance hazardous to health, namely respirable crystalline silica (RCS) and other foundry dusts from your sand reclaim activities which can cause occupational silicosis and lung cancer, is either prevented or, where this is not reasonably practicable, adequately controlled. This is because employees have not been provided with engineering controls and suitable respiratory protective equipment (RPE). IN5/ You have failed to undertake a suitable and sufficient assessment of the buildings on your site to determine if asbestos containing materials (ACMs) are present. Exposure to ACMs can be cause lung conditions, such as mesothelioma. IN6/ You have failed to provide suitable and sufficient facilities for your employees to rest and to eat meals, in that you have not provided adequate number of tables and seating for the likely number of people to use them at any one time. Food and beverages consumed in the workplace are likely to become contaminated, such as by molten metal fume, aluminium dust, Respirable Crystalline Silica dust and other contaminants. IN7/ You have failed to ensure that your local exhaust ventilation (LEV) systems, which are used for the control of substances hazardous to health, namely aluminium dust from fettling and finishing tasks, are thoroughly examined by a competent person at intervals of no less than 6 months, as detailed in Schedule 4 of COSHH. Your previous thorough examination of these systems were undertaken in July 2022, whereby multiple systems were deemed unsatisfactory and no retest was undertaken. IN8/ You have failed to ensure that risk from the exposure of your employees to vibration is either eliminated at source or, where this is not reasonably practicable, reduced to as low a level as is reasonably practicable, in that your employees are undertaking tasks with vibrating tools, such as fettling tasks through use of handheld grinders and sanders without any control measures in place, such as time limits. PN served because there is an accumulation of aluminium dust within the fettling and linishing area, including ledges, floors, surfaces and high level beams etc, in close proximity to potential ignition sources, such as electrics and hot works, giving rise to the risk of fire and/or explosion and you have failed to eliminate or reduce this risk as far as is reasonably practicable
