Description	You have failed to ensure that the exposure to paints containing Isocyanates have been prevented or adequately controlled by providing controls such as Local Exhaust Ventilation (extraction). You have failed to adequately control employees’ exposure to isocyanates, a substance hazardous to health, because you have not enclosed the isocyanate spraying process in a room or booth and are not using local exhaust ventilation to extract the substance at source. You have failed to ensure that lifting equipment, namely your four 2-post vehicle lifts within you workshop, which is exposed to conditions causing deterioration which is liable to result in dangerous situations has been thoroughly examined, in the case of lifting equipment for lifting persons, at least every twelve months You have failed to take suitable and sufficient measures to prevent, so far as is reasonably practicable, any person falling a distance liable to cause personal injury namely by failing to provide suitable guarding on all of the mezzanine floor areas
