IN/MH/151123/02-You have failed to ensure that your local exhaust ventilation (LEV) system, used to control a substance hazardous to health, namely wood dust, was thoroughly examined by a competent person within 14 months. IN/MH/151123/03 - You have failed to ensure that employees who are liable to be exposed to wood dust, a substance hazardous to health that can cause occupational asthma, are under suitable health surveillance. IN/MH/151123/04 - You have failed to ensure that you use an appropriate local exhaust ventilation (LEV) system for your Makita Chop Saw. Your LEV is used to control a substance hazardous to health, namely wood dust, and when you use a vacuum as a type of portable LEV it must of at least a class M (medium).
