| Description | IN AS 1311123-01 (313824442 ) As the mine operator you failed to take the necessary measures to ensure, so far as is reasonably practicable, that the mine and its equipment are designed, constructed, equipped, commissioned, operated and maintained in such a way that persons at work can perform the work assigned to them without endangering their own health and safety or the health and safety of others. I reviewed your safety management system (SMS) to identify the control measures required to secure workers health and safety. The bow-tie assessment that you have used for ‘fire prevention’ was not suitable nor sufficient to identify all the hazards or quantify the risks, and did not provide the necessary control measures for preventative or mitigating barriers. Appropriate Safety Performance Indicators (SPIs) that monitor the continuing effectiveness of control measures, applied to both prevent adverse events and to mitigate harm should an event occur, were not suitable or sufficient. IN AS 1311123-001 - (313824544 ) You have failed to ensure that every work activity, including operation, use and maintenance of a system and work near a system, was carried out in such a manner as not to give rise, so far as is reasonably practicable, to danger, in that you have failed to 1. have the mine’s earth nest tested at suitable intervals the last recorded test being 7.4.19. 2. have the mine’s 3.3 kV system including the surface 250kVA and underground 200kVA transformers high tension and low tension protection relays test tripped and ensured they are set correctly for the system they are protecting, the last recorded test being 4.4.22. 3. have the 95 mm feeder cable earth continuity and conductivity tested the last recorded test being 30.7.20. IN AS 1311123-02 - (313824544) As the mine operator, you have not prepared and kept up to date a suitable written scheme for the systematic maintenance and testing of mechanical plant and equipment at the mine, with a view to ensuring the health and safety of persons at work in the mine. You have not ensured that suitable written reports are made of inspections and that any significant defects identified are recorded, and the steps taken or proposed to be taken to remedy them, have been taken, whilst ensuring that these steps are appropriate. Specifically, you could not demonstrate that periodic instrumented, or speed / distance brake testing was undertaken on underground and surface mobile vehicles. This exposed workers to an increased risk from runaway vehicles operating in roadways with a 1:10 gradient. Your policy titled 'Written Scheme of Mechanical Inspection. Version 3.0', section 5.3. 'Periodic brake testing' states, 'Periodic brake testing of all mobile plant has been identified by risk assessment in the Major Hazard scheme as a significant preventative control and requires that the engineer will establish a periodic brake testing schedule and undertake and record the results of the brake tests on the mine’s mobile plant.' However this was not done. | ||
