You have failed to ensure that cutters available for use on the Jet model JWS - 34KX, Series No. 14053902 spindle moulder are constructed or adapted as to be suitable for the purpose for which they have been provided e.g. ensuring they have limiting projections.
You have failed to ensure that risks to employees and non-employees safety from the presence of dangerous substances (e.g. explosive atmospheres generated in the spray booths) has been eliminated or reduced so far is reasonably practicable, such as by providing ATEX rated lighting.
You have failed to ensure that employees who are or are liable to be exposed via inhalation to wood dust (a substance hazardous to health) are placed under suitable health surveillance where it is appropriate for the protection of their health.
You have failed to ensure that the exposure of your employees to wood dust (a substance hazardous to health), is either prevented or where this is not reasonably practicable adequately controlled, as the tightfitting respiratory protective equipment provided has not been face fit tested to ensure a proper fit, a requirement to ensure that the RPE can provide the level of protection (assigned protection factor) detailed in the technical specification. |
