Description | You have not maintained the spring supports, provided to reduce the likelihood of a hydrocarbon release due to mechanical failure, on the flare pipework since 2013. Therefore, you are unable to demonstrate the flare line has not been subjected to undue vibration induced stresses due to seized and corroded spring supports with open anomalies BB 2013 247, BB 2013 248 and BB 2013 281. Your 2018 Corrosion Risk Assessment identifies mechanical fatigue due to vibration as potential failure mechanism, but the subsequent 2018 Written Scheme of Examination did not identify vibration mechanical fatigue as an inspection requirement. The 2019 examination summary report CC-B-42-00-2-CS-1409337-2019-S01 stated the three flare lines (VF42003, 42008 & 38024) which include the seized and corroded spring supports had been deleted. Therefore, no inspection of those lines had been undertaken. The three seized and corroded spring supports with anomalies 247, 248 and 281 should have been maintained or replaced on several occasions since 2013, but the work has been constantly postponed. An Engineering Query EQ-GA-03557 required the spring supports to be replaced during 2020. But the work was not undertaken. | ||
