You identified failures to your safety critical HVAC system on Leman 27E in 2019, including the Temporary Shelter HVAC fan that provides air movement and pressurisation, and the Battery Room Fire Damper. You did not have a risk assessment in place at the time of inspection in relation to this defect and your maintenance records do not reflect the safety critical status of this equipment, which has resulted in an inappropriate prioritisation of corrective work and repairs have not been completed within 4 years. |
