Description	Our investigation into the incident on the Island Innovator on the morning of 11th June 2023 revealed that you are failing to make a suitable assessment of the risks to the health and safety of your employees, and persons not in your employment, arising out of the conduct of your undertaking in that: (a) Your procedure, Energy Isolation, L3-HSE-PK-023, Rev 6 makes no reference to the risk of stored energy from pressurised systems e.g., compressed air. (b) Therefore PTW-12016 for isolating and working on a compressed air system with approximately 300 bar(g) of stored energy did not make provision for the identification of this hazard, therefore the risk of potential exposure to such stored energy that may occur during the work activity was not assessed. (c) This was further exemplified in the associated Task Risk Assessment (TNA-524 – APV Compressor Maintenance) as it also did not identify this hazard and therefore require ALARP measures to be put in place within isolation certificate (6790). As a result, your employees and persons not in your employment were exposed to an increased risk of a stored energy discharge of 300bar(g) as a single valve isolation was in place.
