Description That during my inspection of the Shearwater installation on 18th to 20th April 2023, I found that your existing arrangements outlined in your Safety Case Issue July 2021 Amendment 03 February 2023 and procedure ‘Operational Risk Assessment for Impaired and Degraded Safety and Environmental Critical Elements (SECES) Doc No. UPO/W/T-MAINT-PR-03’ were either not being followed or are not considered to be appropriate arrangements to operate an installation with impaired safety or environmental critical elements that are required to prevent or limit the effects of major accidents; such as, Category B findings from your Independent Verification Body had not been assessed under the PR03 procedure or sufficient assurance and audit trail on why they had been discounted; mitigations and supporting procedures not sufficiently defined; residual risk not sufficiently defined; findings from risk assessment not clearly defined; approvals not always being undertaken in accordance with the escalation matrix; auditing not effective at picking up issues; no effective procedural loop to ensure reassessment when significant changes occur. Similar issues were also found at my inspection of the Shearwater installation on 17th to 19th August 2022.
