| Description | IN1 You have failed to ensure a suitable and sufficient Asbestos Refurbishment & Demolition Survey has been carried out at the premises prior to demolition and refurbishment work being carried out. IN2 You have failed to ensure all systems are of such construction as to prevent, so far as is reasonably practicable, danger. You have also failed to ensure work activities near your distribution box as well as use of the system has been carried out in such a way as to prevent danger. IN3 You have failed to ensure that each part of the construction site, so far as is reasonably practicable, has been kept in good order & a reasonable state of cleanliness. The site must also be suitable secured and fenced off with suitable signage identifying itself as a construction site and displaying contact details of site management. IN4 You have failed to plan, manage and monitor the construction phase and coordinate matters relating to health and safety during the construction phase to ensure that, so far as is reasonably practicable, construction work is carried out without risks to health or safety. IN5 You have failed to ensure that suitable and sufficient welfare facilities which comply with Schedule 2 of The Construction (Design and Management) Regulations 2015 are provided on site; because you have failed to provide a toilet facility and a sink that provides hot and cold running water in the immediate vicinity of the sanitary conveniences. Suitable & sufficient rest facilities must also be provided on site. IN6 You have failed to ensure that suitable and sufficient measures have been taken to prevent, so far as is reasonably practicable, any person falling a distance liable to cause personal injury. PN You have failed to ensure a suitable and sufficient asbestos demolition and refurbishment survey has been carried out at the premises, and have failed to plan, manage and monitor all works on site, to prevent risk to the health and safety of operatives and others who may be affected by your actions. | ||
