Description	Improvement Notice x 5 served, 1) You have failed to ensure that measures are taken to prevent access to dangerous parts of machinery, namely the rotating table and associated work pieces on your single Webster Bennett 36 inch vertical boring machine. 2) You have failed to ensure that measures are taken to prevent access to dangerous parts of machinery, namely the rotating table and associated work pieces on your single Webster Bennett 60 inch vertical boring machine. 3) You have failed to make and give effect to such arrangements as are appropriate for the effective planning, organisation, control, monitoring and review of the preventive and protective measures for manging health and safety risks from accessing dangerous parts of machinery, including failing to ensure that you have a system in place to ensure that all guards and safety devices on your machines are in place before use. 4) You failed to ensure that your two portable Kemper Local Exhaust Ventilation (LEV) units numbers C182 and C186 provided to control exposure to a substance hazardous to health, namely mild steel welding fume have been thoroughly examined in the previous 14 months by a competent person. 5) You have failed to ensure that lifting equipment, namely your single Scissor Lift has been subject to a thorough examination and inspection by a competent person within the last or 6 months as required for lifting persons.
