Description Improvement Notice 1 - IN/FCA/FNML/14072022/1 You failed to ensure the Local Exhaust Ventilation (LEV) systems provided as an engineering control measure to control employee exposure to substances hazardous to health, including metalworking fluid, an asthmagen, has been thoroughly examined and texted by a competent person within the previous 14 months. Improvement Notice 2 - IN/FCA/FNML/14072022/2 You failed to ensure that your employees, who are liable to be exposed to hazardous substances namely water-mix metal working fluid, which may cause Occupational asthma and work related dermatitis are under suitable health surveillance. Improvement Notice 3 - IN/FCA/FNML/14072022/3 You failed to ensure that the exposure of your employees to a substance hazardous to health, a known asthmagen, namely metalworking fluid, including its use on the CNC XYZ Lathe in unit one is either prevented or, where this is not reasonably practicable, adequately controlled. Improvement Notice 4 - IN/FCA/FNML/14072022/4 You failed to take effective measures to prevent access to the dangerous parts of machinery, namely the rotating lead and/or feed screws of your Hardinge Lathe, Colchester Triumph 2500 Lathe and Colchester Triumph 2000 Lathe which pose risks of entanglement in the rotating parts. Improvement Notice 5 - IN/FCA/FNML/14072022/5 You have failed to make and give effect to such arrangements as are appropriate, having regard to the nature of your activities and the size of your undertaking, for the effective planning, organisation, control, monitoring and review of the preventive and protective measures specifically your guards and other protective measures to prevent access to dangerous parts of machinery for your manual machines. This because a number of fixed guards were absent such as chuck guards on your Bridgeport Milling machines and lead/feed screws guards on your manual lathes. Prohibition Notice 1 - PN/FCA/FNML/1407/2022/1 You have failed to take effective measures to prevent access to dangerous parts of the machinery, namely the rotating chuck and cutting wheel on your Bridgeport milling machine. Prohibition Notice 2 - PN/FCA/FNML/1407/2022/2 You have filed to take effective measures to prevent access to dangerous parts of the machinery namely the rotating chuck of the lathe. Prohibition Notice 3 - PN/FCA/FNML/14/7/2022/3 You have failed to take effective measures to prevent access to dangerous parts of the machinery namely the rotating chuck and cutting tool on your Bridgeport milling machine.
