Description	IN served - You have failed to ensure that the local exhaust ventilation (LEV) system and associated systems, which are provided to control your employee’s exposure to a substance hazardous to health, namely wood dust, have been subject to a Thorough Examination and Test by a competent person within the last 14 months. IN served - You have failed to prevent or adequately control the exposure of your employees to a substance hazardous to health namely wood dust during the use and operation of the Makita DXT chop saw, in particular you have failed to install and operate an effective local exhaust system (LEV) in order to reduce exposures to airborne wood dust, or operate any equally efficient method to control the release of wood dust. You have consequently failed to ensure as far as reasonably practicable the health safety and welfare at work of your employees in particular you failed to manage the risks associated with working with wood and the risk of exposure to wood dust. IN served - You have failed to prevent or adequately control the exposure of your employees to a substance hazardous to health namely wood dust during the use and operation of the Makita LS1013 chop saw, in particular you have failed to install and operate an effective local exhaust system (LEV) in order to reduce exposures to airborne wood dust, or operate any equally efficient method to control the release of wood dust. You have consequently failed to ensure as far as reasonably practicable the health safety and welfare at work of your employees in particular you failed to manage the risks associated with working with wood and the risk of exposure to wood dust. IN served - Employees are exposed to risk of injury as you have failed to take effective measures to prevent access to or to stop the movement of dangerous parts of machinery, on the Chipper machine located in the yard area, namely the rotating chipper cutting heads, before any part of a person can enter the danger zone.
