Description IN 01: You have failed where it is necessary to prevent injury to any person, to take suitable and sufficient steps to prevent, so far as is reasonably practicable, the fall of any material or object, in that you have allowed the boundary fence adjacent to a rail line to become damaged over time by the forces placed upon it, so that there is a risk of items falling over the fence and injuring those working on the rail line or striking a train. IN 02: You have failed to organise your workplace in such a way that pedestrians and vehicles can circulate in a safe manner, in that you have failed among other considerations, to post site rules at the site entrance and provide measures to segregate vehicle and pedestrians. IN 03: You have failed to ensure that measures are taken which are effective to either prevent, or, where that is not reasonably practicable, adequately control exposure of a person using work equipment to any risk to their health or safety from specified hazards, such as articles being ejected from the discharge side of the Lefort box shear.
