| Description | That you have failed to ensure that the risk of exposure of your employees and others to substances hazardous to health (namely legionella bacteria) is either prevented or, where this is not reasonably practicable, adequately controlled in that no written scheme for controlling the risk to legionella bacteria exists for Triton FPSO and that you could not demonstrate prevention and control arrangements detailed in Dana procedure UK-UK-HS-HEA-STD-0004 (Managing Water Quality and risks from Legionella) are replicated in the actual working practices onboard the installation. Specific examples include but are not limited to; Potable Water bunkering hose management compliance gaps, visual internal hot and cold water tank inspections not being undertaken on an annual basis and calorifier temperature control being found out of specification limits. Further to this you have failed to begin to undertake improvement actions recommended within the 2019 Legionella risk assessment. | ||
