Notice 312492044 served against Ithaca Oil and Gas Limited on 11/05/2022
Notice Type Improvement Notice
Description that during my inspection onshore on 23 March 2022 and the subsequent follow up meetings in April 2022 you failed to demonstrate that there are suitable arrangements in place for maintaining the integrity of the installation, including arrangements for - (a) periodic assessment of its integrity; and (b) the carrying out of remedial work in the event of damage or deterioration which may prejudice its integrity in that: 1) You have not carried out periodic structural inspections of multiple cargo tanks on the installation as required by your marine integrity management plan (ITH-AF-579-008, Rev 01) and those inspections are overdue by several years in a hull 9 years past its design life. 2) You have not inspected the cargo tank no. 7 for seven years which had a 615mm crack on the main deck centreline longitudinal girder previously repaired. This has a scheduled inspection interval of 5 years. 3) You have not inspected the port slop tank since 2014 which has a scheduled inspection interval of 4 years. 4) The high H2S content in the cargo increases the potential for corrosion in the cargo tanks. 5) You have not quantified the extent of side shell plating requiring remedial work. But you have noted significant corrosion and wall thickness loss in the side shell plating and by your own assessment the side shell plating will need remedial work such as coating reinstatement and/or additional internal stiffening in some locations to prevent local buckling.
