You have failed to conduct a suitable and sufficient assessment of the risk of such fire and/or explosion created by your shredding process. As such you are unable to demonstrate that your shredding processes and controls of paper dust are safe. Observation on the day of my visit led me to the opinion that dust is not being adequately controlled. This exposes your employees to risk of injury or death from fire and explosion. | ||
