| Description | 27/04/2022/APt1 You have failed to provide appropriate measures to prevent vehicles falling over the edge at the picking belt and at the quarry waste tip IN APT27042022/1 (312509006) No person competent to manage the quarry was present on site on 27/04/22 when I inspected the quarry and no one was nominated as the competent substitute manager for the quarry. I formed this opinion because no benches, haul roads, the picking belt stockpile or waste tips had adequate edge protection in place. The blast design documentation for the 24/03/22 placed the shot-firer and sentries within the danger zone marked on the document. No defects were recorded on the quarry weekly and monthly inspection reports indicating inadequate edge protection. The inspection records had not been countersigned by senior management. IN APT27042022/2 ( 312508974) The Articulated Dump Truck (ADT) driver who was working in the saw shed on 27/04/22, the date of the inspection stated he had had no formal training to drive and operate the ADT on site but had been doing so for several years.The telehandler driver was driving the machine without a correctly adjusted review mirror and an intermittently working reversing alarm. The pre-use inspection document filled in by the telehandler driver did not indicate any defects with the mirrors or reversing alarm. | ||
