| Description | You failed to ensure that your employees or other workers are provided with adequate training on your arrangements to operate installations with impaired safety or environmental critical elements that are required to prevent major accident hazards, in that during my inspection of Cleeton on 15th to 17th February 2022 I found that despite your existing arrangements outlined in your Safety Case Volume 1 PUK-SMS-SC- 001' and procedure 'Guidance on Operational Risk Assessment PUK-SMS-RM-007' these were not being followed in that, on multiple occasions since 2019, employees failed to identify the additional mitigations needed to control the impaired critical element (namely the Temporary Refuge integrity failure identified in May 2018 and further brought to your attention by your external verifiers in 2019); in addition, the inspection has identified similar failings on Minerva and Neptune installations regarding impairment of the Temporary Refuges. | ||
