| Description | You failed to make and give effect to appropriate arrangements to operate an installation with impaired safety or environmental critical elements that were required to prevent major accident hazards, in that during my inspection of the Cleeton installation on 15th to 17th February 2022 I found that your existing arrangements outlined in your Safety Case Volume 1 PUK-SMS-SC-001' and procedure 'Guidance on Operational Risk Assessment PUK-SMS-RM-007' were not being followed in that they did not identify the additional mitigations needed to control the impaired critical element (namely the Temporary Refuge integrity failure identified in May 2018 and further brought to your attention by your external verifiers in 2019) and that there was insufficient monitoring and auditing to ensure that the procedure was being followed and the necessary mitigations were in place or action taken when a failure to follow the procedure or inadequacy of the procedure was identified; in addition, the inspection has identified similar failings on Minerva and Neptune installations regarding impairment of the Temporary Refuges. | ||
