Description | During the inspection of the cargo/slop tanks on the BW Catcher on the 18th-21st January 2022, it was found that on the 7th November 2021 during the gas monitoring of the ullage space of the port slop tank there was a recorded reading stating 'over 1000 ppm H2S' (ppm = parts per million, H2S = Hydrogen Sulphide gas). H2S is a toxic gas that can be rapidly lethal at levels above 1000ppm. At the time of the inspection you were unable to demonstrate that a suitable and sufficient risk assessment had been carried out for H2S and the potential exposure to persons onboard the BW Catcher. Therefore, you have failed to ensure that the measures necessary to prevent or adequately control the exposure of your employees and others to H2S arising from their work have been considered. | ||
