Description IN.WP.HE.210521.01 Failure to ensure that exposure of substances hazardous to health, namely welding fume, has been adequately controlled. This is because your fixed Local Exhaust Ventilation (LEV) system is not capable of reaching many areas where welding is occurring as the adjustable ducting is either poorly located or too short AND you only have one portable LEV unit which is not sufficient to provide suitable control when two or more welders may be welding at the same time. IN.WP.HE.210521.02 Failure to ensure that exposure of employees to substances hazardous to health, namely weld fume, has been adequately controlled as you have not provided employees with Respiratory Protective Equipment (RPE). IN.WP.HE.210521.03 Failure to make arrangements to plan, monitor, control and review the condition of metal working fluids; to ensure you are preventing or adequately controlling the risk of dermatitis to your employees. IN.WP.HE.210521.04 Failure to make arrangements to ensure effective planning, organisation, control, monitoring and review of your preventative and protective measures. There are no effective arrangements in place for checking safety measures, such as guarding and use and maintenance of LEV and that employees are properly supervised.
