Description IN - Notice No: 311489065 Were not providing the necessary information, training and supervision to ensure employees were wearing personal protective equipment including hearing protection where and when necessary. IN - Notice No: 311489128 Your Ex protected electrical equipment was not being operated, inspected, and maintained in line with the manufacturer’s essential health and safety requirements or relevant benchmark standard (BS EN 60079), in that you do not have appropriate arrangements in place to plan, organise, control, monitor and review the specification, design, engineering, maintenance, inspection and modification of your protected electrical equipment and systems, used in those areas of your premises where there is potential for an explosive atmosphere to occur which may therefore give rise to a risk of serious injury or death. Items of Ex protected equipment were found with defects that made them credible ignition sources in explosive atmospheres. IN - Notice No:311489144 The drive shaft on the Greaves mixer fitted to Tank 6 is not adequately guarded so as to prevent access to dangerous parts of the machine which may therefore give rise to a risk of serious personal injury. IN - Notice No: 311489159 There were several areas of none compliance with the British Aerosol Manufacturers Association (BAMA) requirements and guidance for aerosol filling operations on site including, but not limited to, un-protected electrical equipment that is too close and over-sized opening between the external gassing room and the internal operating area. For these reasons, you have failed to reduce risks associated with the aerosol filling line currently operated on site so far as is reasonably practicable. IN - Notice No: 311489201 You have failed to make a suitable and sufficient assessment of the risks to employees which arise from dangerous substances present or liable to be present on site. The existing risk assessment had not, for example identified the likelihood and persistence of an explosive atmosphere, the presence of credible ignition sources in areas processing flammable substances and connected areas such as the one between the gassing house and the rest of the aerosol fill line. PN - Notice No: 311489228 Prohibiting: Processing flammable materials in the 'Atex Area' without adequate risk elimination / reduction measures in place to reduce the risks arising from the generation and handling of flammable vapours during the mixing process to as low as is reasonably practicable. Because: Several items of Ex protected electrical equipment on the mixing vessels in the 'Atex Area' had ignition capable defects in areas where flammable vapours were present in normal operation which, if they became active would result in a fire, fireball, or explosion. PN - Notice No: 311489255 Prohibiting: Processing flammable materials on filling Line 6 without adequate risk elimination / reduction measures in place to reduce the risks arising from the generation and handling of flammable vapours during the mixing process to as low as is reasonably practicable. Because: Filling Line 6 was fitted with un-protected electrical equipment in areas where flammable vapours were present in normal operation which, if they became active would result in a fire, fireball, or explosion.
