Description	IN/JB/11/12/2020/01 - you as an employer and COMAH Operator have not taken all measures necessary to prevent major accidents and to limit their consequences for human health and the environment in that have not ensured that work equipment; namely Tank TF25 has a Written Scheme of Examination that can be used to identify any deterioration such that it can be detected and remedied in good time IN/JB/11/12/2020/02 - you as an employer and COMAH Operator have not taken all measures necessary to prevent major accidents and to limit their consequences for human health and the environment in that have not ensured that work equipment; namely Tank TF31 has a Written Scheme of Examination that can be used to identify any deterioration such that it can be detected and remedied in good time IN/JB/11/12/2020/03 - you as an employer and COMAH Operator have not taken all measures necessary to prevent major accidents and to limit their consequences for human health and the environment in that have not ensured that work equipment; namely Tank TF25 which is exposed to conditions causing deterioration which is liable to result in dangerous situations is inspected at suitable intervals to ensure that health and safety conditions are maintained and that any deterioration can be detected and remedied in good time IN/JB/11/12/2020/04 - you as an employer and COMAH Operator have not taken all measures necessary to prevent major accidents and to limit their consequences for human health and the environment in that have not ensured that work equipment; namely Tank TF31 which is exposed to conditions causing deterioration which is liable to result in dangerous situations is inspected at suitable intervals to ensure that health and safety conditions are maintained and that any deterioration can be detected and remedied in good time IN/JB/11/12/2020/05 - you as an employer have not ensured that person(s) engaged in the following activity, namely the erection and dismantling of tower scaffold(s) are competent to do so or, if being trained, are being supervised by a competent person.
