| Description | IN NB 4667399/1 (311162383) You have failed to ensure that the surface 11 Kv distribution system is constructed as to prevent, so far as is reasonably practicable, danger because the construction of the system allowed on the 20th of November 2020 an electrical fault condition to cause an explosion and subsequent fire in the Bank Back Fill substation and a fire on number 22 PIX switch in the main surface substation IN NB 4667399/2 (311162431) You failed to prevent danger, on your surface 11 Kv distribution system by maintaining it to prevent, so far as is reasonably practicable, such danger; 1. On the 20th of November 2020 the 110 volt tripping batteries in the main 11 Kv surface substation failed to operate the P40 Agile protection relay on number 22 PIX 11 Kv circuit breaker under fault conditions resulting in explosion and fire in the Bank Back Fill substation and fire on number 22 PIX switch in the main surface substation. 2. You subsequently also failed to demonstrate that the 110 volt tripping batteries in the main surface substation and the Sahut substation were inspected , tested and maintained to ensure that they would operate protection relays under fault conditions 3. You also failed to demonstrate that protection relays in Sahut substation were inspected , tested and maintained to ensure that they would operate under fault conditions Both improvement notices complied with see letter reference 4667399/1 and 2 | ||
