This is because you have failed to ensure that employees relying on tight fitting respiratory protective equipment have been face fit tested by a competent person. IN issued as you have failed to have suitable arrangements in place for the monitoring the quality of your water-mixed metal working fluids, namely the levels of bacterial contamination. Exposure to bacteria in metal working fluids can cause ill health including dermatitis and occupational asthma. IN issued as you have failed to ensure that employees that are liable to be exposed to noise levels that will be a risk to health are under suitable health surveillance.
