| Description | 1 x Improvement Notice (PS08102020/1, 310992959) issued:- you have not taken appropriate measures to limit the extent of fire or explosion including measures to prevent the spreading of fires and the communication of explosions from one location to another or protect persons from the effects of fire or explosion in that additional controls had not implemented to prevent the powder drawer blowing out during an ignition on the PC53 machine. 2 x Improvement Notice (PS06102020/2, 310993390) issued:- you have not taken appropriate measures to limit the extent of fire or explosion including measures to prevent the spreading of fires and the communication of explosions from one location to another or protect persons from the effects of fire or explosion in that additional controls had not implemented to prevent the emergency relief chimneys within the filling machines from being left open after maintenance. The use of a padlock is not ALARP. You had recognised there was an issue and that an interlock system was possible as it was on Line 3 Blank filling machine but you had failed to implement this on any of the other machines. 3 x Improvement Notice (PS06102020/3, 310993405) issued:- Every operator must take all measures necessary to prevent major accidents and to limit their consequences for human health and the environment. You have not taken appropriate measures to limit the extent of fire or explosion including measures to prevent the spreading of fires and the communication of explosions from one location to another or protect persons from the effects of fire or explosion. You have identified that in operating the blank capping machine PC238 machine you require the operator to decant HT1 caps onto a mesh tray before being loaded into the machine. You identify in your safety report that there is potential for an explosion on the preparation table when the caps are on a sieve, being prepared to be dispensed by hand into the machine. The live capping machines do not have this issue and have had this process engineered out. You have chosen not to make similar arrangements with this machine. | ||
