Description	IN/SPD100720/01 - 310767815 You have failed to take measures to prevent or, where that is not reasonably practicable, adequately control, the risks to employees safety from the unintended discharge of any article (such as components or connectors) or any gas or liquid (such as the medium being used for testing) during the pressure testing of diving umbilicals which is carried out in the workshop with no means of segregating the operator from the testing assembly in place. IN/SPD100720/02 - 310767835 You have failed to adequately control your employees' exposure to solder fume, which is a substance hazardous to health and a significant cause of occupational asthma, in that; you have not provided adequate ventilation systems, in sufficient number, to control the release of fume at source. IN/SPD100720/03 - 310767844 You have failed to ensure that the fume extraction system; provided to control exposure to isocyanates (which are substances hazardous to health and a significant cause of occupational asthma) during the dispensing of moulding agents, has undergone a thorough examination and test within the last 14 months by a person who is competent to do so.
