| Description | You failed to cooperate with your client, Diamond Offshore Drilling (UK) Limited, the dutyholder for the Ocean Valiant, so far as is necessary to enable them to comply with the duties set out under this regulations, in that you failed to take reasonable steps to bring to their attention, the safety alert issued on 2 February 2016 following an incident in Norway where your service engineers were potentially exposed to asbestos fibres whilst removing the cylinder heads during an engine overhaul. The safety alert made it clear to assume any engines built before 2000 to contain asbestos. The Ocean Valiant was commissioned in 1988 and therefore the engines are most likely to contain asbestos. You further failed to stop the job once it was established that the serial number was found to be close to the serial number of the engines associated with the safety alert and establish by testing that the elbows were asbestos free before commencing the 24k hours service. You further failed to properly cascade the safety alert to your service engineers. On 2 September 2019, as part of the 24k hours overhaul of the ‘Wartsila Vasa’ power generation diesel engine, the night shift crew in the process of removing the air intake elbows in order to replace the worn out O rings, damaged the asbestos insulation, resulting in asbestos debris being dispersed to the surrounding walkway and engine block thereby potentially exposing the crew to asbestos fibres, which later was analysed as containing Chrysotile. Following the incident, you failed to make a suitable and sufficient assessment of the risks to your employees and others exposure to asbestos, in that your updated Job Safety Analysis (JSA -00092287 dated 14/11/2019), failed to highlight the potential risk of asbestos present in engines installed prior to 2000, but depended on a customer declaration at the bottom of the JSA for the site/installation to be free from asbestos and or any other hazardous material. | ||
