You failed to take reasonable steps to ensure that (a) a suitable and sufficient assessment is carried out as to whether asbestos is or is liable to be present in the premises;( b) a written plan identifying those parts of the premises concerned is prepared and the measures which are to be taken for managing the risk are specified in it; and (c) adequate information, instruction and training is given to your employees and others carrying out work where they are liable to be exposed to asbestos. On 2 September 2019, as part of the 24k hours overhaul of the Wartsila Vasa main power generation diesel engine, the night shift crew in the process of removing the air intake elbows in order to replace the worn out O rings damaged the asbestos insulation, resulting in asbestos debris being dispersed to the surrounding walkway and engine block thereby creating the risk of potential exposure of the crew to asbestos fibres, which later was analysed as containing Chrysotile. |
