IN#1 - Company unable to demonstrate that fixed electrical system had been maintained so as to prevent, so far as is reasonably practicable, danger.
IN#2 - Company has not made a suitable and sufficient assessment of risks arising from the potential for exposure to Asthmagenic and Carcinogenic substances hazardous to health, in use at company's premises, including, but not restricted to METAPAS Replenisher F Yellow, SLOTOPAS Z 21 Blue and SLOTONIP 2010A Nickel Solution and SLOTONIP 2010 R Replenisher.
IN#3 - Company has not made a suitable and sufficient assessment of risks arising from the potential for exposure to substances hazardous to health, eg. PC stripper, Stowlin UN2927 which contains Hydroxide and Dichloromethane and Zinc ACYN 102, excluding those which are asthmagenic or carcinogenic and subject to IN/RJB/ 02/04/20/02.
IN#4 - Company has failed to ensure that employees exposed substances which are hazardous to health, including, but not restricted to SLOTONIP 2010 A and SLOTONIP 2010 R Replenisher, which are respiratory sensitisers, are under suitable health surveillance.
IN#5 - Company has failed to provide a suitable record of the thorough examination and testing of the local exhaust ventilation system for the caustic soda cleaning tank in the Anodising workshop provided for the control of substances hazardous to health.
IN#6 - Company has not ensured that the Hydrovane Compressor and HPC Receiver, in use at its premises has been thoroughly examined by a competent person within the intervals specified in a written scheme of examination.
IN#7 - Company has not appointed one or more competent persons to assist in undertaking the measures needed to comply with the requirements of the relevant health and safety statutory duties.
PN#1 - Prohibition of work activities in location of the distribution board and consumer unit in zinc shop (front unit), which are being carried on by company at the premises above, which will involve a risk of serious personal injury, and that the matters which give rise to the said risks are that covers are missing, giving access to live parts of the distribution board and consumer unit, because operatives working in the vicinity can come into contact with live parts.
PN#2 - Prohibition of work activities in the vicinity of the consumer unit just outside the bath area in zinc shop (front unit), which are being carried on under company's control at the above premises, which involve a risk of serious personal injury, and that the matters which give rise to the said risks are that the cover is missing, giving access to live parts of the consumer unit, because operatives working in the vicinity can come into contact with live parts.

PN#4 - Prohibition of work activities in the vicinity of the rectifier in zinc shop (rear unit), which are be carried on under company's control at the premises above, which will involve a risk of serious personal injury, and that the matters which give rise to the said risks are that the cabinet housing cannot be closed, giving access to live parts of the rectifier, because operatives working in the vicinity can come into contact with live parts. |
