| Description | Vertellus Specialties UK Limited - 5 x IN - (ECI & ME) - 30.03.20 IN 1 - R J M /VSUK/2020/02 - Failure to take all measures necessary to prevent COMAH major accidents as you do not have in place adequate procedures and instructions for implementing a functional safety managements system for the site’s safety instrumented systems. IN 2 - R J M /VSUK/2020/03 - Failure to take all measures necessary to prevent COMAH major accidents as you cannot demonstrate that the safety instrumented systems on plant 2 (DCDPS process) have the necessary level of integrity. IN 3 - R J M /VSUK/2020/04 - Failure to take all measures necessary to prevent COMAH major accidents as you do not have in place adequate procedures and instructions for managing the risks from electrical installations in potentially explosive atmospheres to ensure the risks are reduced as far as is reasonably practicable. IN 4 - R J M /VSUK/2020/05 - Failure to take all measures necessary to prevent COMAH major accidents in that you have failed to ensure that fixed and moveable electrical installations and systems in potentially explosive atmospheres are maintained in an efficient state, in efficient working order and in good repair to ensure that risk is reduced so far as reasonably practicable.
