| Description | Serial No: IN2 4 0220SW1 - Notice No:310464395 The COMAH Competent Authority inspection of your premises on 11th February 2020 revealed that you had not taken all measures necessary to prevent major accidents, in that you did not have appropriate arrangements in place to plan, organise, control, monitor and review the inspection and maintenance of your protected electrical equipment and systems, used in those areas of your premises where there is potential for an explosive atmosphere to occur. Serial No: IN2 4 0220SW2 - Notice No:310464448 The COMAH Competent Authority inspection of your premises on 11th February 2020 revealed that you had not taken all measures necessary to prevent major accidents, in that you were not adequately maintaining the Basic Process Control Systems (BPCS) in place on your premises to keep the processes under control. Electrical drawings were not available, wiring within the main BPCS panel was hanging out of the trunking, un-terminated cables were present, equipment was not identified, and no maintenance records existed on a system claimed as providing a layer of protection against several major accident hazard scenarios. IN - Serial No: CEMHD-CB040620-01- Notice No:310683922 The COMAH Competent Authority inspection of your premises on 11th February 2020 and subsequent inspection by correspondence revealed that you are using electrical equipment in the SLF Low Voltage system that is of inadequate strength and capability and may give rise to danger. | ||
