Description	You have failed to make and give effect to such arrangements as are appropriate, for the effective organisation, control and monitoring of the preventative and protective measures noted below. My investigation into an incident on the 10/01/19 where one of your contracted employees was required to leave the installation for medical treatment due to suffering from an alleged work-related eye condition established that the requirements of the following procedures were not implemented: OCH-WI-001, Operating Procedures Offshore, Rev 1, Issue 01/08/18, an Iqarus UK Limited procedure implemented by the Offshore Medic on your behalf requires that a documented consultation is completed for a patient by an Offshore Medic that could have led to further consultation with a topsides doctor and possible medical evacuation with onward support to a medical facility. PL-STAN-HSSEIA-011, Incident Reporting, Classification, Investigation and Analysis Standard, requires that all work related incidents shall be reported, recorded, and investigated to determine root and contributory causes, and determine corrective and preventative actions to protect the health, safety and welfare of other employees. As a result, the employee left by routine flight two days after initially raising the incident with the OIM (08/01/19), had to use their own onward transport, and had to arrange follow up medical care via a family member; furthermore no evidence was provided of an investigation into the incident at the time.
