| Description | As requested, you recently supplied us with a Predictive Analysis of your sites COMAH Major Accident Hazards. This analysis was: 1. Limited in detail given the extent of the hazards present on site, and, 2. Significantly less sophisticated than a Predictive Analysis from several years ago from our own records of your site. Having discussed this with you, I therefore conclude that there has been a loss of Corporate Memory by F2 Ltd as a result of senior staff changes in recent years. This also leads me to conclude that your current Safety Management System is deficient with respect to the identification and evaluation of major hazards: the adoption and implementation of procedures for systematically identifying major hazards arising from normal and abnormal operation, including subcontracted activities where applicable, and the assessment of their likelihood and severity. | ||
