For this reason, you have failed to take all measures necessary to prevent major accidents and to limit their consequences for human health and the environment. 2) Notice: IN100919SW2 - 310108621 The LOPA (Toxic) indicated a SIL 2, PFH 6.85x10-7 (high demand mode) Safety Instrumented Function (SIF) requirement to protect the Road Tankers from over-fill. This safety-related trip was in place but was not being managed in line with BS EN 61511. For this reason, you have failed to take all measures necessary to prevent major accidents and to limit their consequences for human health and the environment.
