Description Improvement Notice 309712274 (IN VF NRB CPL 01) issued as I identified that you failed to prepare and keep up to date a suitable written scheme for the systematic inspection, maintenance and testing of all electrical plant and equipment at the mine, with a view to ensuring the health and safety of the persons at work at the mine, in that not all 415 and 230 volt equipment on the surface was included on your written scheme. For example the Baldwin and Francis 415 volt Board involved in the incident involving Tom Dixon was not on your written scheme and no test and commissioning records were available for it, in addition the MICOM electrical fault protection relay on the board was not wired in, so was not operating. Similarly an English Electric 415 volt Board in the workshop substation whilst being on your written scheme was not being systematically maintained, as no maintenance schedules were being issued for it. These examples and others seen could lead to danger of electric shock or fire, from equipment malfunction. Improvement Notice 309712382 (IN VF NRB CPL 02) issued as my investigation revealed you failed where necessary to prevent danger, by the use of suitable means, including, where appropriate, methods of identifying circuits at 415 and 230 volts on your extensive surface system, because labelling ranged from satisfactory to illegible, or absent. In addition where line diagrams of circuits at these voltages were posted they were out of date and inaccurate, and there was a general absence of line diagrams. All of which could lead to confusion and danger from risks of electric shock. Improvement Notice (IN VF NRB CPL 03) issued as my investigation revealed that whilst specialist surveys have been carried out to determine the energy levels at various points within the mines surface electrical network to identify the risks of Arc flash, the Baldwin and Francis Board involved in the incident was not included. In addition calculations supplied post incident for equipment in the substation create doubt over the arc flash energy stated in warning notices displayed on the Qualter Hall 415 volt Board. In recognising the Arc flash risks and as part of the arrangements for reducing risks, special arc flash PPE had been issued to electrical personnel, but not to Mr T Dixon the injured person. The combination of these put persons working on electrical equipment at increased risk
