Description Failed to demonstrate to the competent authority that you have taken all measures necessary to prevent major accidents and to limit their consequences for human health and the environment in that following identification of three sections of corroded small bore pipework on the Ethylene import line near control valve BR-002 you have failed to ascertain the remaining well thickness at corroded areas and use that information to verify that the risks associated with a leak of ethylene occurring are reduced to as low as is reasonably practicable. Failed to take all measures necessary to prevent major accidents and to limit their consequences for human health and the environment in that you do not have an effective integrity management system that is proportionate to the hazards and ensures that you manage and control the risks associated with ageing equipment installed in the establishment and it's corrosion.Prevention should be considered in a hierarchy based on the principles of reducing risk to a level as low as is reasonably practicable (ALARP). There were instances at your site including pipework in COMAH duty with extensive external corrosion scale, where it would have been reasonably practicable to determine the actual damage present and degradation rates and mechanisms of the equipment rather than relying on problematic analyses to determine fitness for service.
