| Description | The IP and an agency forklift truck (FLT) driver were working together to empty two small tip-bins into larger commercial waste bins. The tip-bins were placed on a pallet transported by the FLT to the waste area and the IP was standing on the edge of the pallet. The IP got his foot caught in a strap and as he tried to shake it off, his leg got even more tangled. The IP fell onto the ground and the FLT went over his ankle. | ||
